- Three month commitment required for the direct patient care health membership; month to month thereafter.

- Early cancelation membership fee $100
- If by provider discretion it is determined that we are unable to provide safe and quality care at an app

- As the healthcare world has not caught up with our awesome direct patient care health membership just yet, we recommend you carry insurance in case of an emergency. Ally Medical Health Membership does not have a hospital or surgical center to incorporate flat rate medical costs with at this time.
- Psych/Mental Health, Hormone Replacement, and Weight Loss visits are not included in the membership.
